Overview:

By 2030, demographic shifts project that one in five Americans will be 65 years of age or older. This dramatic increase in the aging population poses significant challenges, especially for healthcare systems already stretched thin. A crucial concern is the widening care gap when patients are discharged from hospitals and enter home-based care, where care is often provided by family members who are untrained in healthcare practices. This transition is fraught with risks, including poor management of chronic conditions and higher rates of hospital readmission and ED visits.

Addressing this care gap is more than a public health imperative—it's also an economic one. This is an opportunity not only to improve care, but to improve resource utilization, decrease returning ED visits, and extend the capabilities of your workforce as well. This webinar offers a timely and critical solution: the strategic inclusion of unpaid caregivers as vital extensions of healthcare teams. By doing so, we not only bridge the chasm of home-based care but also infuse healthcare organizations with a distinctive competitive advantage. In an industry where differentiation is key, the role of informal caregivers could be the decisive factor that sets your organization apart from your competitors.

Who Should Attend?

Healthcare providers, administrators, and insurance professionals will find invaluable insights in this webinar, focusing on turning caregivers into essential assets for population health management. Attendees will walk away with actionable strategies to effectively incorporate family caregivers into the healthcare model, thereby enhancing patient outcomes and establishing a competitive edge for their organizations.
